Address:You may have obtained PAN number years ago when living in India. Address on the PAN record could still be showing old address in India. To update PAN with overseas address, use the application form on this page to request changes.

Name:Women may choose to change their surname after marriage. Individuals may have made a few changes to their name recently. In such case, request Income Tax Department of India to update their database with correct Name.

How to apply for PAN Changes and request reprint

Applicant must choose carefully what changes are needed on duplicate PAN Card (example - address, photo, etc)
Name Change can be made to applicant surname or first name on PAN Number and PAN Card.
Also, other changes include address, date of birth, photo, and e-mail address.
Form CSF is used to request Income Tax Department for corrections. We help create your Form CSF after payment.
Documents are required to establish proof of ID, proof of Date of Birth, and proof of Address.
PAN Card will be delivered to Overseas address - if foreign address proof is provided.
If the old PAN Card is lost, applicant can use Form CSF to request for duplicate printing of PAN Card.
For women with change of surname after marriage, applicant may request to update name during request.

(a) Marriage Certificate
(b) Copy of passport showing husband's name
(c) A Deed Poll executed with lawyer mentioning both old name and new name
(d) A Statutory Declaration executed with lawyer mentioning both old & new names
(e) Publication of name change in official gazette
(f) A Certificate issued by a gazetted officer

Individual Applicant other than married ladies

(a) A Deed Poll executed with lawyer mentioning both old name and new name

Send signed application documents by post/courier

Signed application documents are physically required at our India office to begin processing. Applicants must send signed documents to our Chennai address by Postal Service or Courier - with tracking.

Option 1: Applicants in USA/Canada may send documents to our New Jersey address (Additional postage fee is required).
Option 2: Applicants send documents directly to our Chennai address in India at their own postal expenses. We will provide you the address after completing document reviews over e-mail.

For applicants who opted for Option 1 and paid additional postage fee, our office in New Jersey uses FedEx to forward the documents to India as per dispatch schedule displayed on the website.
